FORM: Disbursement Request
Use this form to submit reimbursement requests for ministry expenses. Remember, you should always get a purchase pre-approved by your ministry leader or a staff member to ensure reimbursement. Please submit this form with documentation for the purchase (receipt) to the Eternal office.

All Compass Kids / Youth teachers and volunteers are required to have background checks. Please print and complete this form and submit to your ministry leader.
